STEP 1 - Getting Started | |
Contact your local Service Learning Team (SLAC Reps and Service Learning Champion) to consult about the application process, discuss project objectives and requirements, and create a project development timeline. | |
STEP 2 - Approval Process | |
1. | After consulting with the local Service Learning Team, Students complete appropriate sections of the SLAC application then submit it to their local Service Learning Team and their Faculty Advisor for review. |
2. | The local Service Learning Team reviews the application and makes recommendations for moving forward. Students incorporate feedback, secure a Faculty Adviser signature, and email the signed copy to the local Service Learning Team. Upon local approval, a designated SLAC Regional Representative (SLAC Rep or Service Learning Champion) will sign off on the application, then forward it to the Service Learning Team in Seattle (somserve@uw.edu). |
3. | The Seattle Service Learning Team may perform additional reviews and follow up, as needed, then prepare the application for review and approval at a future SLAC Meeting. |
4. | Project applications are reviewed by the Service Learning Advisory Committee (SLAC). If changes are requested, students will implement changes and resubmit. |
5. | Once approved, the Assistant Dean for Student Affairs reviews the recommendations of the SLAC and the Service Learning Manager and issues a decision. |
Updating your SLAC Application | |
If the scope of your project has changed in any way (i.e. additional services, new site with identical protocols, etc), your SLAC application must be updated and re-submitted for approval by the SLAC. Document updates via the SLAC Application Renewal Form Download SLAC Application Renewal Form. Examples of changes in scope include, but are not limited to, the following: new target population, adding or omitting any lab testing, adding services, adding or changing site/community partner. If you have a new outreach idea that will fall under an existing project (i.e. a new outreach site that offers a different scope of care), please complete a new SLAC application, but label it as an ADDENDUM. Please report changes of scope to the Service Learning Team, at somserve@uw.edu. |
