This assignment will guide you through a structured review of your teammates and your own participation on your team, and your team's overall effectiveness working together.
Your review and comments for each team member will be provided to them. These comments will be anonymous - your team members will not know who gave what comments. Likewise, you will receive anonymous feedback from your team members.
Your grade for this assignment is based only on completing the evaluation - your team members' comments will not be used to determine your grade, nor will your comments determine their grade for this assignment.

Please note the following:
- You will fill out the entire survey separately for every member of your team, including yourself! E.g. If you have six team members, you would fill out the survey six separate times.
- All parts of the survey are required. You will not receive full credit if you do not answer each of the questions.
- You should receive an email confirmation with your answers, which you can use to edit your responses if desired. If you do not receive an email confirmation, your responses did not go through and you should resubmit them.
- Because you and your teammates need this information to complete their final participation assessment, and due to the time required for me to send out the data, I will not accept late submissions of the surveys after the due date.
